Perfil microbiológico y resistencia antibiótica en infecciones urinarias de gestantes: Un estudio transversal en un laboratorio clínico privado de Jaén, Perú

Determinar el perfil microbiológico y patrones de resistencia antimicrobiana en gestantes con infección del tracto urinario (ITU) en un laboratorio clínico privado de Jaén, Perú. Estudio descriptivo y transversal (n=110), realizado entre septiembre 2025 y enero 2026. Se identificaron uropatógenos mediante pruebas bioquímicas y susceptibilidad por Kirby-Bauer (CLSI). Se detectaron betalactamasas de espectro extendido (BLEE) por sinergia de doble disco. Se utilizó ANOVA para evaluar la resistencia según variables sociodemográficas y obstétricas. La prevalencia de ITU fue 54,55% (n=60). Predominaron bacilos Gram negativos (83,33%), siendo Escherichia coli el agente más frecuente (63,33%), seguido de Klebsiella pneumoniae y Proteus mirabilis (8,33% c/u). En cocos Gram positivos (16,67%), destacó Staphylococcus aureus (6,67%). Se identificaron cepas multidrogorresistentes productoras de BLEE. Se halló una elevada resistencia global a ampicilina (83,33%) y alta sensibilidad a nitrofurantoína (90,00%). El análisis ANOVA no mostró diferencias significativas en la resistencia según edad (p=0.320) ni trimestre gestacional (p=0.412). La elevada prevalencia de ITU y la detección de cepas BLEE exigen fortalecer la vigilancia epidemiológica local. La nitrofurantoína se confirma como una alternativa eficaz para el manejo empírico en la población estudiada.
